New Patients

We encourage you to download our New Patient Form, print it out and bring it along with you on your first visit to the office. In order to maintain your privacy, please do not send the information through email.

Finding Our Office

We are located approximately fifty miles North of the Golden Gate Bridge, and 3 miles from US 101. We have a map and detailed directions to our office as well as tips on where to stay if you are from out of the area. If you need further help with travel or lodging plans, please call us at 707-526-2276.

As of April 1st, 2009 the Cosmetic Fee Plan financing plan is no longer being offered by Captial One. We regret the inconvenience and have begun seeking out other arrangements on behalf of our patients for the future.
